Patient Level Reports

Clinicians can generate patient-specific reports directly from the Patient Profile within Connected Health. Reports may include a single vital sign or survey score, multiple selected metrics, or a combined report containing several vital signs and survey scores.

1. Accessing Patient Reports

  • Navigate to the Patient Profile.
  • Select the Reports tab.

2. Configuring Your Report

2.1. Select Metrics

Use the Metric drop-down menu to choose the data you want to include in the report.
You may select one or multiple metrics.

Tip: Only Surveys with Scoring will be available. Survey reports display survey scores only, not the individual question responses. Use this option when reporting standardized assessments such as CAT Scores.

2.2. Set the Date Range

Enter the From and To dates to define your reporting period. Reports can be generated for a single day or multiple days.

2.3. Add Notes *optional*

Add any notes to the report that you want visible on the report. 

Tips

  • Use the Notes field on the report generation page to add up to 500 characters of context directly to a Vital Signs or Survey Score report.
  • Notes entered are included only in the report being generated and are not saved to the patient record.
  • The Notes field resets after each report is generated.
  • If no notes are entered, the report will display a blank Notes section for handwritten annotations if needed.
  • If you add a new note or modify an existing note after generating a report, simply select Generate again to refresh the report and include your updates.

2.4. Generate the Report

After selecting your metrics, date range, and have added an optional note, click Generate Report.

Tip: The generated report automatically includes a monthly average for each selected outcome.

2.5. Download the Report

To download the report, click the Generate Report button.
A PDF file will be saved to your computer.
