New Patient Profile View and Onboarding Process

This article will guide you through the process of onboarding a patient to Cloud DX Connected Health by using Cloud DX's new Patient Profile View and Registration Process.

To comply with your organization's guidelines, please do not begin this process until after the patient has consented to participate.

1. Create the Patient Profile

To create the patient account, from the main clinical dashboard select the Patients Tab and select New Patient.

To complete a new patient account, add the relevant patient information to the form. All required fields have a red star.

Please ensure that when adding phone numbers, you enter the country code first. The format is +1-area code-phone number.

If desired, use the drop downs to add additional Patient Demographics and additional Contacts:

Demographics: 

This section allows the clinical team to add additional demographic information, including the patient's mailing and shipping addresses.

If you're using Cloud DX Drop Shipments, it's essential to expand the Demographics section and add the patient's accurate shipping address when creating the patient profile.

Patient Contacts/Extended Care Team: 

The Patient Contacts/Extended Care Team section enables the clinical team to log additional contacts relating to the patient, such as a physician, emergency contact, or caregiver. To add new patient contacts, select the + sign.

Select the contact Type from the drop down and fill in the applicable fields.  Once your contact is saved, you can continue to add additional contacts by selecting the + sign

Required fields when adding a Patient Contact: 

Once you have completed the patient profile, click Save

2. Register the Patient

Registrating the patient for a username and password is the next step once the patient's profile has been created. This will enable the patient to log in on the Connected Health Application and connect their vital signs/participation to your clinical portal.

To Register the Patient, select Register Patient from the bottom of the patient profile: 

Create the Username and Password

When you register the patient, you will see a Register Patient pop-up. There are two options: auto-generate the username and password, or manually create them yourself.

(Recommended) Autogenerate Username and Password: 

If enabled, the platform will autogenerate the patient username and password based on agreed upon formula. 

If the auto-generate username/password option is grayed out, this means that it has not been configured for your clinic. Contact your clinic administrator or Cloud DX DST representative to learn more about this feature. In the interim, follow the instructions below to create a manual username and password on behalf of the patient.

Manually Enter Username and Password:  Enter the desired username and temporary password for the patient. 

OPTIONAL- Send New Email Verification to Patient: 

For the patient's account to be verified, they can be sent an email verification that will guide them to verify their email. To do this, select "Send New Email Verification to Patient," then add the patient's email address. Once you click "Register" they will be registered for an account and will receive an email similar to the one below:

Once you have completed the patient registration, select Register. You will be redirected to the patient profile, and a green bar will confirm that the account has been successfully registered.

3. Request a Welcome Call or Drop Shipment

After the patient is successfully registered, you can request either a welcome call or a drop shipment. To do so:

  1. Select the Quick Actions drop-down menu from the Patient's Profile.
  2. Select Welcome Call or Dropshipment
  3. Fill out the form that appears with all the applicable information:
    • Request Type: Welcome Call or Drop Shipment
    • Date: Entered the patients preferred date for a Welcome Call or the date that you would like Cloud DX to initiate the Drop Shipment.
    • Notes: Add special notes relating to the Welcome Call or Drop shipment. For example, preferred time of day (2 hr block at minimum), if a caregiver support will be available, to activate the tablets Data, ect..
  4. Select Save. A green bar will appear confirming the Welcome Call or Dropshipment Request has been successfully sent to the Cloud DX Support Team. 

If there are any issues with the Welcome Call or Drop shipment Request, the Cloud DX Support or Deployment Team will be in touch via Email.
